what's up with this?

Doctors Without Borders states on page 7, 23% of those surveyed in Afghanistan, somehow, have had a friend or family member die from violence in the past year alone, and 87% of the violence was alleged to be from the current conflict. How did this happen, who was involved? How does this effect the perception of US foreign policy, and the US in general, after 12 years of being there?
